New In Vitro Techniques
Author: Michael Kamrava, M.D.
The new simplified techniques of in vitro fertilization (IVF) include NORIF, SCORIF, TUFT and others. Because there are many causes of infertility, not every procedure is right for everyone. Each couple's history and situation must be carefully evaluated to determine the most appropriate procedure for them. The following are general answers to the most often asked questions about the advances in IVF.
WHO IS A CANDIDATE FOR THE NEW PROCEDURES?
Couples with a variety of infertility disorders, such as tubal disease, endometrioses, male factor, unexplained infertility and immunological infertility.HOW DO THEY WORK?
As pointed out above, there are a number of related new techniques. To keep the explanation simple, we will explain how the sister techniques of NORIF and SCORIF are performed. In both NORIF and SCORIF the egg(s) is(are) removed by ultrasound aspiration from the ovaries. This procedure is done in the doctor's office. The egg is mixed with sperm and placed in a small plastic dish and left in the incubator for 2 days. The fertilized egg(s) is(are) then transferred to the uterus through a small plastic catheter.
The difference between NORIF and SCORIF is that no hormones are used with NORIF. The egg released during ovulation as part of the woman's natural cycle is retrieved in the doctor's office by ultrasound aspiration.
For some women -- for example, those who do not ovulate naturally -- SCORIF is the more appropriate approach. With SCORI , hormones are used to stimulate a menstrual cycle. For some women, TUFT or one of the other approaches, is better.
HOW DO THE COSTS COMPARE?
The new procedures cost about one-fourth to one-half the average cost for standard IVF, and some insurance companies are covering this cost.HOW DO THE SUCCESS RATES OF THE NEW PROCEDURES COMPARE WITH THE SUCCESS RATES FOR STANDARD IVF?
Success rates compare very favorably. With standard IVF, the success rate is approximately 20%; NORIF 8% and SCORIF 20%. The lower rates with NORIF are due to a single egg recovery from natural cycle versus multiple eggs with SCORIF.WHAT ARE OTHER ADVANTAGES OF THE NEW PROCEDURES?
There is no need for use of blood components in these patients. Patients benefit psychologically because they are able to participate actively in the process. The procedures are performed in the privacy and convenience of the doctor's office, so the patients are able to follow a normal daily routine throughout the IVF periodFor more information, contact Michael Kamrava, M.D. via e-mail to: wci@ix.netcom.com
